  • Scenarios not always in line with recommendations

    It’s a great little Ap to catch a quick review, but be cautious as some of the treatments in the scenarios are not in line with current recommendations. For example, the scenario with cat scratch disease (Bartonella) recommended withholding antibiotics despite the patient having over a week of fever and lymphadenopathy. The scenario also recommended a lymph node biopsy with PCR testing to confirm the diagnosis despite a very clear history. The research on antibiotics vs no antibiotics is pretty clear and the expert consensus is that any patient with a clear history and lymphadenopathy be treated with azithromycin. The consensus is that lymph node biopsy should be reserved for cases that do not respond to appropriate antibiotics. UpToDate has a great review of cat scratch disease.

    This app takes you step-by-step through various patient encounters almost like a game, which is a fun change-up from just reading. When simply reading about a topic, it’s hard to retain everything. There’s also a difference between understanding the material and knowing exactly how to put it into practice. This app help you learn the material AND grasp how to apply it in clinical settings. They already show lab results, but I haven’t seen imaging results, and that would be helpful. Other nice additions would be more questions about what the differential diagnosis is and what history questions and physical exam components you’d add. Basically, more questions would help me think even more critically through the process. Thanks for a fun, helpful app!

    I wrote this a little ago “Please add more pain related scenarios, I want to be a pain medicine doctor. So please add some where you can prescribe opioids and other non opioid painkillers I’d really like that.” You guys said you would but haven’t I’m kinda of disappointed, that’s why I won’t be giving it a 5 star but other than that it’s a really good game and teaches you a lot.

    The consequences you face when you select a real time response are cleaver. I like how I can retry to select the correct modalities and diagnosis after I witness what happens when I made the wrong choice. Plus it looks up to date, so I can share this with my school and not worry about it.

    Just started medical school, but have been playing with this app for a few months now. Usually in the morning while I have a coffee I run through a case, while simultaneously searching through UpToDate in order to select the most correct answers and to learn more about a specific process. While I don’t know for sure if all of the information is 100% accurate (I’m not a doctor, yet) it usually falls in line with the recommendations I find. This app provides a case-based way to search through interesting topics and begin to apply some very basic clinical reasoning skills. For example, I just saw a case of acute pyelonephritis, and read about the clinical presentation, diagnosis, treatment recommendations, prognosis, and long-term follow-up of pyelonephritis and urogenital anatomical abnormalities on UpToDate!

    [Update: changing my rating to one star as there’s been no progress on this issue for months.App crashes 100% on my iPad Pro within seconds of launching.I had a friend with an iPhone 7 try it and it crashes too after a few pages into a scenario.] Great game, although new scenarios are sometimes slow to arrive. But the crashing soon after launch which stated happening within the last month makes it unplayable. Please, please fix.

    I made it about 2 minutes into the first scenario before the app went black and I was forced back to the home screen. Now, every time I try to open it it does the same thing after about 10 seconds. Please update this so it works!

    في الحقيقه اجد الكثير من المتعة خلال استخدام هذا التطبيق لما فيه من محاكاة واقعية للقصص واتخاذ القرارات وشرح كل خطوه بالتفصيل، قد تاخذ منك وقتا لإنهاء القصه لكنك ستخرج بمعلومات قيمة ان شاء الله، شكرا لمن قام بانشاء البرنامج ولكل من ساهم فيه. Quite amusing to be honest I really do enjoy every story they provide us with a detailed cases and choices that test your knowledge, so thank you for putting such an effort into this app we really appreciate it.

    First, this app needs to be updated so it can work with updated devices instead of having to turning off the phone to get it to stop. However, the app itself is quite long with the stories! I do think that it’s good to have a way to show a physician interacting with patients and not just running in and out. But realistically, one gets to the main point and then moves on. I much prefer the Prognosis app, which gives you the info such as medical history, current complaint, and vital signs, etc., which is more like what you’d do at a real appointment. Then you can still make decisions about tests, meds, surgery, or whatever. It has a lot of information about the different diagnoses as well, and discussions at the end sometimes. Hopefully, this will change some, will be updated, and maybe I’ll try it again. Try the Prognosis apps if you haven’t!

    This App has been a very good learning tool. A lot of different cases and options for testing if need be. There's a broad spectrum of cases so it doesn't get boring.I have learned a lot of medical terminology and variations in different test counts.
